FIRST VISIT:
Your initial appointment will consist of a 3-dimensional scan of the tooth and x-rays. From there, Dr. Sulim will perform a series of tests on the tooth and explain the diagnosis as well as the treatment options.
In some cases, a consultation will be required prior to treatment being scheduled. Those cases consist of: examination of multiple teeth, possible crack/fracture of the tooth or if the tooth has had a prior root canal on it.
What to bring to your initial appointment:
- Driver’s license/photo ID
- Insurance card
- Copy of referral form (if applicable)
- List of medications
If you are a new patient to our office please arrive 15 minutes prior to your appointment time for paperwork. If you wish; we can also text you a link to the new patient forms to fill out prior to your appointment. If you have any questions regarding your first visit please give us a call.
