FINANCING YOUR CARE

As a courtesy to you, we will contact your insurance company to verify insurance benefits and calculate your estimated out-of-pocket cost. We will make every effort possible to inform you of your estimated cost prior to your appointment.

Please Note: Estimate is due in full at the time of service.

IMPORTANT: Due to varying insurance policies, the complexities of insurance contracts and services that may have been billed prior to your visit that may affect your yearly maximum, we can only ESTIMATE your coverage in good faith based on the information provided to us by your insurance company.

The cost provided to you at the date of service is only an estimate and never a guarantee of benefits. All treatment performed will be billed to insurance.

Once your carrier has paid the claim, you will be responsible for any difference. If you have a dispute regarding payment, it will be your responsibility to contact your insurance provider to file a dispute.

For your convenience we accept Visa, MasterCard, Discover and American Express. You may also pay with cash or check.
